what if kung nalukot, nabasa or medyo nagkaroon ng damage ang PRC form ko, tatanggapin pdin ba yun ng PRC?

do we need to include the name and signature of scrub nurse?? kasi sa school nmen hnd na ni-require.. and im done with my prc forms.. i-su2bmit ko na lang to sa prc and if ever na totoo to uulit uli ako??!

i am a graduating nursing student this april and planning to take the board exem on november. just want to clear something about the form to be used for the cases, there are lots of form that was issued but they are different but they all claiming that it was the latest just want to ask to please give me a copy on my e.mail the real latest form for the cases. Were on trouble arguing about it so please help us to clear things about it. Thanks

Can you please send me the form that i will have to use? I will be taking the June board. I believe they released a new form. Am i supposed to use that or this old one that you have on your site? Please e-mail me the correct forms for all cases. Thank You!
