What: Application for Admission to the June 2009 Nurse Licensure Examination
When: Until April 17, 2009 only.
Where: Central Office, P. Paredes Street near Morayta
(also at its satellite accepting center at the 2nd Floor of Ever Gotesco Mall – Recto Branch) in Manila or at any regional offices located in the cities of Baguio, Cagayan de Oro, Cebu, Davao, Iloilo, Legazpi, Lucena, Tacloban, Tuguegarao, and Zamboanga.
Requirements for Application:
- Duly accomplished Nurses’ Application Form (NAF)
- NSO issued Birth Certificate
- NSO issued Marriage Certificate (if applicable)
- Transcript of Records with scanned picture and reflecting the remarks: “FOR BOARD EXAMINATION PURPOSES”
- Duly notarized Record of OR-DR Cases (for first-time applicants only)
- Duly notarized Summary of Related Learning Experience (for first-time applicants only)
- Four (4) passport size (1½ x 1 ½ ) colored picture with complete name and white background
- Recent Community Tax Certificate (CTC)
- P900.00 examination fee
- One (1) window mailing envelope
