CHECKLIST FOR EVENING CARE

  1. Explain the procedure and gather the equipment.
  2. Do oral care.
  3. Loose the top sheet at the foot part of the bed and remove the gown.
  4. Do partial bath.
  5. Position patient to prone, expose back and place towel at the side.
  6. Sponge back and proceed to back massage by using the different strokes.
  7. Bring back patient to supine position.

Checking for Perineal Care

  1. Gather equipment; follow with external douche or perineal flushing. Explain the procedure to the patient.
  2. Provide privacy and position the patient to dorsal. Offer bedpan.
  3. Drape patient properly.
  4. Place the perineal care tray (Flushing tray) between legs. Pour lukewarm water over the vaginal area.
  5. Use cotton balls soaked in water, clean midline in one down ward stroke.
  6. Start from the father side, use cotton balls with soap. Clean vulva gently towards the anus then the inner labia to the outer labia by using the figure of “7” stroke. Use only a cotton ball at a time for each stroke then discard. Do the same at the nearer side.
  7. Rinse vulva by using cotton balls with water in the same manner as in step #13.
  8. Pour lukewarm water and dry the area using dry cotton balls following the same manner.
  9. Clean the groin and thigh, starting at the father thigh from inner to outer direction. Use the same procedure in cleansing. Pat dry with towel.
  10. Remove bed pan and flushing tray and turn patient towards the nurse.
  11. Clean the buttocks and the anal region using cotton balls with soap, then with water for rinsing. Pat dry with towel.
  12. Fix patient (place napkin if necessary). Tidy the area and do the after care of the equipment.
  13. Loosen the bottom sheet, remove crumbs and stretch the linens.
  14. Move to the other side and do the same.
  15. Turn pillow (change if necessary). Comb the hair.
  16. Clean the table and provide water for drinking.
  17. Place the signal button within the patient’s reach. Turn off the light it the patient desires.
